Former House Speaker Nancy Pelosi stunned many Americans this week by openly vowing to fight for nationwide protections allowing sex-change surgeries and hormone treatments for minors.
Speaking Thursday after touring the San Francisco VA Medical Center, Pelosi reviewed health-system upgrades, research projects, and—according to her office—a supposed “threat” from House Republicans to reduce veterans’ services.
When asked about California’s recent pauses in so-called “gender-affirming care,” Pelosi did not hold back:
“That is something I’m working for at the national level, and we are hoping we can have gender-affirming care for our trans kids,” she said, calling the situation “really sad” and admitting she’s uncertain how much can be done with President Trump and a Republican Congress in control.
Pelosi pointed out that a transgender pride flag is displayed outside her office and mentioned that several fellow Democrats showcase the same symbol. She admitted that a large number of Republicans strongly oppose medical transitions for minors but insisted she will continue to push her agenda.
Clash With the Trump Administration
Her comments come as over a dozen Democrat-led states have filed a federal lawsuit against the Trump administration to block restrictions on sex-change procedures for those under 19.
Filed in the U.S. District Court for the District of Massachusetts, the lawsuit lists President Donald Trump, Attorney General Pam Bondi, and the Department of Justice as defendants. It contests Trump’s January 28 executive order blocking federal backing for such procedures, as well as DOJ instructions issued by Bondi and Assistant Attorney General Brett Shumate.
Bondi’s memo orders the DOJ to investigate and prosecute medical providers who perform sex-change procedures on minors. Shumate’s memo prioritizes federal cases against doctors, hospitals, and pharmaceutical companies involved in such treatments.
National Impact Already Felt
Trump’s action has already led to sweeping changes:
- Kaiser Permanente announced it will halt sex-change surgeries for patients under 19 beginning August 29.
- Children’s Hospital Los Angeles shut down its Center for Trans Youth Health and Development, one of the nation’s largest clinics for transgender minors.
- Children’s National Hospital in Washington, D.C., also ended gender-transition medical services for underage patients.
Currently, many states have passed laws restricting or banning such procedures for children. But the Democrat-led states in the lawsuit—California, Connecticut, Delaware, Hawaii, Illinois, Maryland, Massachusetts, Michigan, Nevada, New Jersey, New Mexico, New York, Pennsylvania, Rhode Island, Washington D.C., and Wisconsin—still allow them.

Pelosi’s pledge sets the stage for a major national showdown over whether minors should have access to irreversible medical procedures. With President Trump and Attorney General Bondi committed to enforcing strict protections for children, the issue is shaping up to be one of the most heated cultural battles in America today.
